CARES 360 Community Clinic:
A natural evolution.
In 1985, Nashville CARES began with a mission born out of urgency, compassion and grit: to care for our neighbors living with HIV, and to fight the surrounding stigma. Over the years, that mission has grown into a movement. We've evolved from crisis responders to leaders in public health, equity and innovation. CARES now provides dignity, acceptance and unwavering care for 20,000+ individuals across Middle Tennessee each year.
As the scope of our mission continued to expand, it became clear that this life-changing work would be even more effective if we could reimagine our clinic as a separate (but closely related) entity. That dream is now a beautiful reality. By establishing CARES 360 as a full-service health equity provider, the CARES family is already setting a new standard for inclusive, community-based healthcare.
But this new chapter isn’t just about us. It’s about everyone who believes that empathy-based healthcare is a right, not a privilege. It’s about building a Tennessee where no one slips through the cracks. A community where all healthcare is holistic. Where dignity leads the way.

With over a decade of healthcare experience, Rachel approaches patient care with knowledge, empathy and compassion. A board-certified Physician Assistant with Master’s Degrees in both Social Work and Public Health, she’s deeply committed to the biopsychosocial model of healthcare. This approach recognizes that physical health is closely intertwined with mental, emotional and social wellbeing.
Rachel is also credentialed as an HIV Specialist by the American Academy of HIV Medicine. She understands how overwhelmingly complex it can be to navigate the healthcare system. Her clinical practice is grounded in the belief that quality care involves active listening, shared decision making and a holistic view of unique individual circumstances. Working in community medicine has been a lifelong dream, so Rachel views it as a privilege to serve our diverse patient population.

Zach has long been driven by the desire to bring high-quality healthcare to underserved communities. This focus guided him to earn his Master’s Degree in Nursing with a focus on Adult Geriatric Primary Care from Vanderbilt University. There, he found that while he was passionate about serving that population, he also felt a strong pull to work with disenfranchised members of the community. That led to volunteering at vaccine drives in unhoused citizen encampments.
After graduating in 2022, Zach joined a Chattanooga community health clinic, working with patients seeking care for PrEP, PEP, HIV, HCV and primary care concerns. Now that he’s on the CARES team, he continues to expand on his specialized skills and experience, embracing new care and treatment advances as they become available. Is it possible to change the world? For Zach, helping to save lives is a great way to start.

Jewyl Gibson, DNP, FNP-BC, PMHNP-BC
Jewyl Gibson, DNP, FNP-BC, PMHNP-BC Psychiatric Nurse Practitioner
Board-certified as both a Family Practice Nurse Practitioner and Psychiatric Mental Health Nurse Practitioner, Jewyl brings over 19 years of experience serving adults, children and adolescents. Her diverse background includes roles in Family Practice, Internal Medicine and Integrative Medicine. She previously served as Clinical Associate Professor in the University of Tennessee, Knoxville’s graduate nursing program, and as Associate Director of Clinical Operations at United Healthcare.
Jewyl holds a Doctor of Nursing Practice (DNP) from the University of Alabama, Birmingham, and a Master of Science in Nursing (MSN) from Tennessee State University. She also earned a BSN from Tennessee State University and a Bachelor of Arts in Biology from the University of North Carolina, Charlotte. Jewyl is an active member of the American Psychiatric Nurses Association and American Nurses Association.

Amy Fisher, RN
Amy Fisher, RNClinic RN- HIV Engagement and Retention in Care Specialist
Amy brings over 19 years of specialized experience to CARES 360. Her clinical expertise includes direct patient care and patient education, along with implementing holistic strategies for the management of chronic conditions. Previous experience in hospice and palliative care equipped her with a strong foundation in care coordination, symptom management and family-centered education. She comes to us from Vanderbilt Health.
Driven by a deep compassion for those facing complex health challenges, Amy is committed to supporting patients through collaborative, person-centered care. Her work focuses on bridging communication between patients, providers and community resources to ensure comprehensive, high-quality care. As a certified AIDS Nurse through the Association of Nurses in AIDS Care (ANAC), Amy plays a vital role in coordinating care for individuals living with HIV/AIDS
Leslie Reese, RN

With over 30 years’ nursing experience – most recently at Vanderbilt Health – Leslie specializes in the care of medically complex patients, including those living with chronic conditions such as HIV. Her professional journey spans a wide range of healthcare settings, including pre- and post-operative care, home health, inpatient and outpatient services and nurse case management. This wealth of experience adds a deep and diverse clinical background to her role.
Leslie is known for her seemingly endless compassion, and for her unwavering dedication to patient advocacy. Her goal is to break down barriers to care access, empowering patients to maintain their health and enjoy better quality of life. She’s passionate about helping people navigate the complexities of the healthcare system, always working to ensure they receive the care, resources and support they need to thrive.

May Chen, PhD Research Director
May is a public health leader and senior scientist with over 15 years of experience. She’s deeply committed to promoting evidence-based strategies that effectively and equitably foster safe, stable, and supportive environments where all individuals can thrive. May has dedicated her career to advancing trauma-informed care and improving health outcomes within diverse communities.
Before joining CARES, May served as a lead behavioral scientist in the National Center for Injury Prevention and Control at the Centers for Disease Control and Prevention (CDC). There, she provided scientific guidance and oversight over a large portfolio of research. May also served as the Science Lead of the Minority Health and Rural Health Team for the CDC’s COVID-19 pandemic response. She has authored over 50 peer-reviewed journal articles, technical reports and book chapters.

Throughout her career, Elizabeth has been guided by a deep commitment to helping and advocating for people. This passion goes back to childhood, when she witnessed firsthand the stigma surrounding HIV. At her local Boys and Girls Club, one boy was shunned by many because of his HIV status. Two notable exceptions who showed kindness and empathy? Elizabeth and her brother.
That experience shaped a lifelong dedication to building a world where HIV is defined not by stigma, but by compassion and understanding. At CARES 360, Elizabeth is proud to be part of a team working toward that vision. She brings 27 years of healthcare experience to her role as Patient Care Lead at CARES 360, where she is honored to walk alongside patients in their own healthcare journeys. Elizabeth does whatever she can to ensure that every patient feels seen, respected and supported.

Abby began her healthcare journey in 2020, providing COVID-19 testing during the height of the pandemic. That life-changing experience showed Abby two truths about herself – she realized that she had a natural passion for community healthcare, and understood how important it is to care for the whole person. Abby went on to become a Certified Medical Assistant, joining CARES 360 in 2023.
In this role, Abby is dedicated to creating a welcoming, supportive environment for every patient. She values the opportunity to care for people from all walks of life, knowing she may see them on their best days, their worst days and everything in between. Abby strives to ensure that every patient knows they have someone in their corner – an ally who genuinely cares, and is cheering them on through every step of their healthcare journey.
